The Demo Scenario

Kestrel Ridge Mine (KRM) runs a planned crusher shutdown in August 2026. Two contractors work the same corridor at once — which is the entire point.

Code What it is
TORR / KRM-C-2026-014 Torrens Mechanical — maintenance contract, risk tier HIGH
DELTA / KRM-C-2026-021 Delta Electrical — separate contract, same window
APEX Apex Rigging — subcontractor, CONDITIONAL_APPROVAL, may only work under Torrens supervision
WP-2026-0117 CR-200 primary crusher liner change — ACTIVE, HIGH risk, SIMOPS
WP-2026-0121 Substation 3 cable termination — the overlapping package
IF-0117-02 The lifting interface — crane loads pass over the walkway to Substation 3. Risk EXTREME
CTL-01 The critical control: hard barricade + spotter whenever a load is suspended
FA-2026-0442 Control verification that found CTL-01 not in place with a load suspended
INC-2026-0233 Near miss — packer fell ~4 m into the area that should have been barricaded. Actual LOW, potential CRITICAL
DEC-0117-01 → -02 Lifting decision superseded after the stop-work: walkway now closed entirely
TM-TR-04 Service truck rejected at plant inspection — illegible load chart. Never mobilised
PR-2026-Q3-TORR Quarterly review — safety NEEDS_IMPROVEMENT, schedule EXCELLENT

The thread worth following: IF-0117-02HZ-02CTL-01FA-2026-0442 → CRITICAL finding → stop-work → DEC-0117-02CA-2026-0501/0502. That is the core chain end to end, with a real failure in the middle.

Status Vocabularies (the important ones)

Work package: PLANNED → RISK_REVIEW → MOBILISATION → READY_TO_START → ACTIVE → PAUSED / SUSPENDED → COMPLETED → HANDOVER → CLOSED, plus CANCELLED

Contractor: PROSPECTIVE → PREQUALIFICATION → CONDITIONAL_APPROVAL → APPROVED → ACTIVE → SUSPENDED / EXPIRED / DISQUALIFIED / INACTIVE

Control: PROPOSED → ACCEPTED → IMPLEMENTED → VERIFIED, plus INEFFECTIVE, SUSPENDED, RETIREDINEFFECTIVE is a first-class state, not a deletion

Finding: OPEN → ASSESSED → ACTIONED → AWAITING_VERIFICATION → VERIFIED → CLOSED, plus REJECTED, VOID Finding type includes POSITIVE_OBSERVATION — the register is not only a fault log

Action: OPEN → ACCEPTED → PLANNED → IN_PROGRESS → BLOCKED → COMPLETED → VERIFIED → CLOSED, plus CANCELLED, OVERDUE

Competency: REQUIRED · SUBMITTED · VERIFIED · CURRENT · EXPIRING · EXPIRED · REJECTED · NOT_APPLICABLE Access: REQUESTED · UNDER_REVIEW · APPROVED · ACTIVE · SUSPENDED · EXPIRED · REVOKED · DEMOBILISED

Plant: PROPOSED · DOCUMENT_REVIEW · INSPECTION_REQUIRED · APPROVED · ACTIVE · RESTRICTED · QUARANTINED · REMOVED · REJECTED

Inspection outcome: PASS · PASS_WITH_ACTION · FAIL · STOP_WORK · NOT_APPLICABLE Escalation: CONTRACTOR_SUPERVISOR → SITE_REP → CONTRACT_OWNER → SITE_MANAGER → SSE → EXECUTIVE → REGULATOR

Rules of Thumb

  • An interface names both sides: work_package_id and other_work_package_id. If only one is populated, the interaction is with an area or a standing hazard rather than another package.
  • A control without a next_verification_due_at is a control nobody is checking. Sort by that column to find them.
  • Competency, induction and access expire independently. All three must hold for someone to be genuinely mobilised — the demo has a supervisor whose competency expired, so access was suspended while the induction stayed valid.
  • Plant approval is per site, per period, per area. RESTRICTED and REJECTED plant may still exist in the register — that is the point of keeping the contractor's asset separate from this site's decision about it.
  • A stop-work must state conditions_for_restart. Restart is an approval by a named person, not a lapse of time.
  • Corrective actions from a performance review are the mechanism for systemic issues; actions from findings and incidents are the mechanism for specific ones. Both land in the same list.
